A leading provider of supports to individuals with disabilities, ESL has physical locations in 8 regions of Louisiana and our array of programs include; support coordination (case management) for children, adults, and aging participants, a wide range of behavioral health and housing supports, early intervention programs for infants and toddlers, and The Chartwell Center for children and young adults with Autism in New Orleans.

We are seeking mission-driven individuals to join our team, who will help us support our participants as they work to achieve their goals.
Our Support Coordinators (case managers) who serve the OCDD (Office of Citizens with Developmental Disabilities) population, serve a caseload of about 35-45 participants.
These participants live in their homes and communities and receive support (which can be physical, medical, educational, or vocational) to assist them to remain in their desired settings.
The primary role of the Support Coordinator is to plan, organize, implement, and monitor the in-home and community based services and support the participant receives.
All new hires complete an initial training program of about 6 weeks.
During this time you will learn how to plan, schedule, and coordinate services to meet our participants' needs.
Learning about the OCDD service system and requirements, making monthly calls, quarterly visits, and annual CPOCs (Comprehensive Plan of Care) are all responsibilities our SCs have, along with maintaining any pertinent medical and health information for your assigned participants.
Support Coordinators must be agile and responsive in their ability to meet the changing needs of participants and service provision requirements.
Support Coordinators are also responsible for the support of participants throughout emergent situations and all public emergencies including weather related evacuations.
While this position is a hybrid office and remote work, for the first 90-day you will be required to be in the office full time.

Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
